Each student will help to write their individual care plan within 6 months of starting at Laineys Care Farm.

The Individual care plan contains important information such as health, medication and how the individual would like to be supported, it also gives each student opportunity to discuss their own abilities, hopes and aspirations to then set realistic goals with staff which is documented in the care plan and reviewed 6 monthly.

The care plan is monitored regularly to ensure students and staff are working to their own targets and achieving their goals.
